The Grundy County Health Department offers a menu of low-cost health screenings for individuals who are uninsured or underinsured. Tests are self-pay, insurance billing is not available for
this service.
All results are sent directly to you. GCHD provides two copies; one for you and one for your provider.

Individual Test Descriptions

Lipid Profile (fasting required)
Blood draw to measure the cholesterol levels in the blood. This test includes total cholesterol, HDL cholesterol, triglycerides, LDL cholesterol, chol/HDLC ratio, and non-HDL cholesterol.

Glucose only (fasting preferred)
This is a laboratory test to check your blood sugar (blood glucose) levels.

Comprehensive Metabolic Panel (CMP) (fasting preferred)
This test is to check liver enzymes, kidney function, electrolytes, glucose, etc.

CBC (Complete Blood Count)
This test is to check platelet levels, red blood cell count, white blood cell count, etc.

Ferritin
A ferritin test helps your doctor understand how much iron your body stores.

Hemoglobin A1C
This test measures your average blood glucose over the last
3 months.

PSA (prostate specific antigen)
This test is intended for MEN only. NIH recommends this test for men age 50+ unless risk factors are present, then it is recommended for younger men.

TSH (thyroid stimulating hormone)
This is a thyroid screening test.

Thyroid panel
This test is a breakdown of thyroid hormones. This is usually only performed on individuals with a known thyroid condition. This test includes T3 Uptake, T4 (thyroxine), and free T4 Index. A TSH is also recommended along with the thyroid panel.

Vitamin B 12 (Cobalamin)
This test checks your Vitamin B12 (Cobalamin) levels in the blood.

Vitamin D (fasting preferred)
This test checks your Vitamin D levels in the blood.
